Sinus lifting

When the maxillary sinus is pneumatized and there is atrophy of the alveolar process of the upper jaw in the area of the chewing teeth, an operation is required to raise the floor of the maxillary sinus for the subsequent installation of dental implants. This manipulation is a form of pre-prosthetic surgery to improve the quality and quantity of bone in the posterior jaw area.

A preoperative assessment of the maxillary sinus is necessary for the success of this precedure. If the amount of bone between the crest of the alveolar process and the bottom of the maxillary sinus is insufficient (<5 mm), then an open procedure for lifting the bottom of the sinus is indicated.

CBCT is used for preliminary assessment and planning

 

The average height of the available bone in the toothless jaw was classified into three classes:

1 — residual bone height of 10 mm in patients with a toothless jaw of 5 years.

2 — residual bone height 5-10 mm, usually found in patients with a toothless jaw 5-10 years old.

3 — bone height is 0-5 mm, as a rule, in patients with a toothless jaw for more than 10 years.

As mentioned above, it is recommended that sinus augmentation be performed in

Classes 2 and 3. If a 10 mm implant is planned, augmentation should be considered in these categories.

In 2003, Lundgren et al. reported spontaneous bone formation in the maxillofacial sinus was reported three months after the removal of the intrasinous cyst, after the sinus membrane had to be lifted.

In 2007, Thor et al. placed sinus implants were installed without the use of autografts, claiming that the titanium surface of the implants provides sufficient thrombogenicity in activating the coagulation system and platelets and stimulating cell growth. Thus, studies have shown that placing implants in the sinus without transplant materials can stimulate the formation of new bone tissue in the sinus cavity.

Thus, sinus lifting in Yerevan, Armenia, can be performed both with and without grafts, depending on the degree of necessary elevation of the floor of the maxillary sinus.
